Insurance Information
The Nebraska Spine Center is a provider for many insurance plans. Check with your insurance carrier to determine if we participate in your plan.
If your spine care is covered by insurance, you will need to give us the name of your insurance company and your insurance ID card. We will make a copy for our files.
Our billing department will assist you in completing your health insurance claims at no charge. Questions about insurance coverage should be directed to our billing department.
If you have a change in your insurance coverage or more than one health insurance policy (and want claims filed with both), please let us know.
There is a nominal charge for other types of forms and requests for duplicate x-rays. These charges are payable in advance of the service.

Auto Insurance
If you have been injured in an automobile accident, portions of your bill may be covered by auto insurance.
We will submit claims to your auto insurance carrier on your belhalf, if you provide us with:
- Insurance adjuster's name and phone number
- your claim number

Payment for Services
It is customary to pay for medical care the day it is received. This includes any co-pay that may be required by your insurance plan. Unless an HMO, PPO or Medicare plan covers you, please pay for office visits at the time of service.
For your convenience, we accept cash, check, Mastercard or Visa.
The Nebraska Spine Center charges interest at a rate of 1.5% per month on the unpaid balance on your account if it becomes 60 days overdue. Collection procedures are initiated to collect payments of all delinquent accounts.
